This form should be used to provide a complaint about the services received by a Service Provider identified through the Resource and Support Hub (RSH). Your complaint will be received by the Capacity Development Team at the RSH. The Capacity Development Team will discuss your complaint with the Service Provider and can withhold your details where requested. The Capacity Development Team will let you know what actions the RSH is taking as a result of your complaint.
